Fees vary based on the services that you receive. Payment is due at the time the service is rendered, including your co-pay. You have several options for making your payment.

We accept most major insurance providers. If you have a question about whether or not your insurance is accepted, you may contact us prior to scheduling your appointment.
We require that you give us a minimum of 24-hour notice if you need to cancel or reschedule your appointment.
For your first appointment, please plan to arrive 15 minutes early so that you may complete your paperwork. This paperwork must be done before you can see the healthcare provider. Please be prepared to provide information on your health, including your allergies, past and current medical conditions, current medications, etc.
You may also download the forms and complete them prior to your first visit or submit them online.
Many insurance plans require a referral from your primary care physician to see a specialist. There are some plans that do not have this requirement. You should check with your insurance company to be sure.
